Consultation Period, Consent to Engage in Treatment, Treatment Plans and Active Patient Status
Determining whether treatment in this practice is the right treatment for you is important. I conduct a thorough evaluation during the initial interview, which will guide treatment with regards to psychotherapy, medication management or both. This initial comprehensive assessment will explore your primary concerns, current sources of stress, psychiatric and medical histories and other information. In some cases, a(n) additional session(s) will be scheduled to complete this initial evaluation (e.g. for a complicated presentation or extensive psychiatric history). Our initial sessions (intake assessment and up to 3 follow-up sessions) together will constitute a consultation period during which we will both assess whether I am the right practitioner for you. I will do my best to suggest alternate psychiatrists if either of us feels that we are not a good fit.
At the conclusion of the initial assessment, we will review treatment options and determine which methods are most appropriate. Individual psychotherapy usually occurs at a mutually agreed upon time with a 45 minute weekly session, but the exact session length and frequency may vary based on your specific needs. If medications are used, they will require more frequent follow-up appointments initially and during times of dose adjustments (i.e. every 1-2 weeks), with follow-up appointments at a minimum of every 12 weeks once stable or in remission.
Because of the nature of psychiatric care, a patient must be seen at least every three months to be considered an active patient of this practice.
Practice Policies | Megan Dankovich, M.D., PLLC
Welcome! I look forward to working with you. The general policy goals of this practice are to help facilitate quality patient care and minimize the burden of tasks that fall outside the scope of direct patient care. I appreciate your careful review of these policies. You may wish to bookmark this page for your reference.
I am considered an out-of-network provider with respect to insurance coverage. This means I do not directly bill any insurance company or third party payer. Please note that you are responsible for full payment of the session fees. If you plan to use your insurance benefits, you will be provided a standard invoice that you may submit to your insurance company for reimbursement. Many insurance companies provide substantial reimbursement of mental health services by an out-of-network provider. I recommend you contact your insurance provider for the details of your out-of-network mental health benefits.
Cancellations and Missed Appointments
My policy is to charge for missed appointments. You must provide a minimum of 48 business hours notice if you wish to cancel a scheduled appointment or you will be charged the full fee for the missed session. Business hours are considered the weekdays between Monday and Friday and exclude holidays. Please note that insurance companies often do not reimburse for missed appointments.
In exceptional circumstances such as travel or inclement weather, I will attempt to facilitate regular appointment intervals by scheduling a phone or Skype session. Please note that your privacy cannot be guaranteed during such sessions. In addition, your reimbursement by insurance may be drastically reduced or limited.
Payment is due at the time of service. Direct payment from your bank may be submitted via a secure, online payment system. Finance charges are assessed for bills overdue by 30 days or more. Appointments cannot be scheduled if an account balance exceeds $400.00, except in urgent situations. Accounts that are 60 days past due may be referred to a collection agency. You are responsible for all attorney fees and court costs if your account is referred to a collection agency.
